Waking up when your hearing aids are out overnight
Short answer: the hours you're least able to hear are the hours your aids are out, which is precisely when the alarm has to work. The reliable answers replace sound entirely โ a bed shaker, a wrist band, or a strobe โ rather than trying to be loud enough to get through.
This is a specific problem that general alarm advice skips entirely. Someone with mild-to-moderate hearing loss who manages conversation perfectly well during the day can be functionally without hearing at 6am, because the devices that bridge the gap are in a drying box on the dresser.
Why sleeping in them usually isn't the answer
It's the obvious workaround and it's generally a poor one:
- Moisture. Aids are vulnerable to humidity and sweat, and overnight in a warm bed is a bad environment. Most manufacturers and audiologists advise removing them, partly so they can dry.
- Physical pressure. A behind-the-ear aid pressed between your head and a pillow is uncomfortable and can cause soreness or irritation of the ear canal.
- Feedback. Lying on an aid frequently causes whistling, which wakes you at the wrong times.
- Battery. Wearing them overnight means they may not last the following day.
- Ear health. Occluding the canal for 24 hours a day gives it no chance to ventilate.
Some people do sleep in them, particularly for safety reasons if they live alone and want to hear smoke alarms. That's a reasonable decision to make with an audiologist, not a default.
The options that actually work
Bed shakers. A pad under the pillow or mattress, wired to a bedside clock. Moves the whole bed, so it doesn't rely on precise contact with your skin, and delivers the most force of any consumer option. This is the standard recommendation for significant hearing loss and the most reliable single answer. The trade-off is that it wakes anyone else in the bed and doesn't travel. Wrist-worn vibration bands. Vibration against the skin, contained to the wearer. Better where you share a bed, or travel, or don't want bedside hardware. Less raw force than a shaker, and it depends on the band staying snug overnight.
Strobe alarms. A bright flashing light, often built into a bedside clock. Works well for people who rouse to light, less reliably for very deep sleepers, and unhelpful if you sleep facing away or use an eye mask.
Combination units. Loud alarm, strobe and shaker together. The belt-and-braces option, and what most specialist retailers will steer you toward if you have significant loss.

The thing people underestimate: redundancy
If missing a wake-up has real consequences โ a shift you can't be late for, a flight, an exam โ one device is a single point of failure. Two independent methods with different failure modes is the standard advice from people who've been caught out: a mains-powered shaker plus a battery wearable, so a power cut doesn't take out both.
This matters more when you can't rely on hearing a backup that someone with typical hearing would catch.
Practical points
Test it properly before you rely on it. Set it for the middle of the afternoon while you're napping, not for the morning of something important. A device that wakes you when you're lightly dozing may not when you're in deep sleep at 4am.
Charge or check nightly. Whatever you use, it has to be part of the bedtime routine. Battery-powered devices with multi-day capacity are less demanding here than something needing a daily charge.
Consider the smoke alarm question separately. Waking to an alarm you've set is a different problem from waking to an emergency you haven't. Specialist smoke alarms with shakers and strobes exist, and in the UK your local fire service may fit them free โ worth asking.

Where a wrist band fits
